This is real and not Photoshop. I saw it myself last year while on a train in Osaka. I didn't know it was an expressway though. From the angle on the train I couldn't see cars and thought perhaps it was some kind of train station. (I would not have guessed an expressway.)

There's a place in New York where the highway goes directly underneath some apartment buildings as well.

The building is the Gate Tower Building and is located in the Fukushima district of Osaka, and the road passing through it is the Ikeda Route of the Hanshin Expressway. The Expressway passes through the 5th through 7th floors and is listed as the tenant of these floors on the information map at the entrance to the building.
